Transition Care for Teens

Each year, dozens of young adults with cleft and craniofacial (CCF) differences age out of pediatric care and are left to navigate the complex adult healthcare system alone. This initiative addresses this critical gap.

The Need: Currently, no organization is dedicated to servicing the needs of these young adults, jeopardizing their long-term health and well-being. Access to funding for speech, social work, and specialized dental services are significant barriers.


The Solution: We are creating a transitional clinic with a dedicated team of practitioners educated in adult CCF care. This program will provide continuity of care and develop clinical guidelines and patient resources to ensure a seamless transition.
